We would like to take a moment to let you know what you may expect from services at the Immaculate Heart of Mary Counseling Center (IHMCC).

First, you should know that beginning with your first contact with us, we include your special intentions in our daily morning prayers and weekly masses. This will continue even after the completion of your treatment. May Our Lady look with tender mercy upon each of her children here below!

When you contact our office, either by phone or email, you will be greeted by one of our front office staff. This staff member will collect some general information from you regarding reason for requesting services as well as general demographic information (i.e., name, address, phone/email, date of birth, insurance information).

At this time, our office will prepare a packet that will be sent to you either through postal mail or via email. Included in this packet you will find a number of informational forms and one that we need to have returned before scheduling your first appointment.

The Client Information Questionnaire is one we ask that you please complete and return to us. Due to the possibly sensitive nature of the information enclosed, we asked that it be mailed or personally delivered to our office in the John XXIII Diocesan Center on 3700 Sheridan Blvd, Ste 1, Lincoln, NE 68506 attention Jillian Janousek. Our director uses this questionnaire to select the counselor best suited to your individual needs, so please ensure it is accurate and complete. Once your counselor has been assigned, our billing department will check your insurance coverage. We will contact you with that information and at that time schedule your first appointment.

Also included is a copy of our Notice of Privacy Practices, Code of Ethics, and Patient Services Agreement. Please review them carefully. You will be asked to sign an Acknowledgement of Receipt of Privacy Practices at your first session. Your signature on this Acknowledgement confirms that you have received and reviewed these documents and agree to their terms.
